Dues and fees : Men's and Women's teams 2007-08 annual CSA dues and fees      (This is a PDF form.)

Note: On the decision of both executive committees, we have replaced the separate men's and women's dues and tournament entry forms with one common form covering all yearly CSA fees except for the individuals.

[Fees for both men's and women's teams and tournaments are now collected by one treasurer, Jamie King. Payments may ONLY be made by check.]

Should you require separate invoices or separate checks for separate items (i.e. if they are under different budgets) simply print multiple copies of the form and use one copy for each budget item.
